Support Coordination
Support Coordination is a service that is funded by the NDIS to assist you to implement your NDIS plan. We offer experienced Support Coordinators who understand the NDIS; keep up to date with the changes; and have a good working knowledge of the local service providers in your area.
The role of a support coordinator is to help you to find the services and providers you need to help you to meet the goals you have written in your plan. We work with you to increase your understanding of how the NDIS works; and how to utilise your funding to get the most value for money for your plan.
• We talk with you about what you would like your services to look like.
• We help you understand the rules of the NDIS.
• We explain how the funding in your budget can be best used.
• We discuss with you how and when you would like to have your services.
• We talk with you about all of the services that are available in your area (and some not so close if suitable).
• We support you to connect to the service providers you choose.
• And we help with any crises/ issues if they arise.
• And a few other things…
Essentially our role is to teach you how to navigate the NDIS and the service system yourself – really to work ourselves out of a job!
Rise Wellbeing Support Coordinators
Here at Rise Wellbeing we have a team of Support Coordinators who have been in the disability and allied health sectors for many years. We work as a team and share new knowledge to ensure that whilst you may have one Support Coordinator, you have access to our collective knowledge.
Rise Wellbeing staff have multiple roles and to eliminate any potential conflict of interest for you, your Rise Wellbeing Support Coordinator is only able to assist you to implement your plan.
Support Coordination is charged as per the current NDIS price guide.
We will enter into a service agreement with you based on your plan, assist you to implement your plan and work towards meeting your goals.
